Others may begin their treatment journey instead in an inpatient treatment program or in any of a number of levels in between. There are several reasons that people with eating disorders may need a higher level of care at some point in their recovery journey including medical instability, suicidality, and need for more structure and support around meals. Outpatient treatment is the most flexible level of care for eating disorders, and its the level of care offering the least disruption of your regular day-to-day life. Its also usually the most cost-effective. You can keep up with school, work, and other commitments while attending treatment appointments. What Happens in Outpatient Eating Disorder Treatment? The cornerstone of your treatment plan is usually individual counseling sessions. On an outpatient basis, your psychotherapy typically occurs at least once a week, with each appointment lasting about an hour. However, depending on your individual needs and the severity of your eating disorder, your therapist may recommend that you attend sessions more frequently. As treatment progresses and you need fewer sessions, your therapist will likely recommend spacing sessions out to every-other-week, and then to once-per-month and eventually to an as-needed basis. Many people also attend family therapy sessions as part of their eating disorder treatment. Your primary therapist may coordinate these sessions, or you may also have a separate family therapist on your treatment team. Your age (adolescent or adult), how far away your family lives, and how involved you are with your family are all factors that will help determine whether these sessions are scheduled regularly or interspersed throughout treatment. If you have a spouse or significant other, he or she may also be encouraged to take an active role in your treatment. Other Types of Therapy Group therapy or support groups are also very common for those in outpatient treatment. However, its important to talk with your primary therapist about when it would be appropriate to start a group and what group she or he would recommend if that is something you are interested in. Group therapy can be extremely beneficial in treatment. However, it can also be overwhelming and be triggering to someone who is new to recovery. Nutrition counseling or consultation with a dietitian is typically recommended as part of outpatient treatment as well. These appointments typically last thirty minutes to an hour and may occur every week or less frequently. They encompass nutrition education, meal planning, and accountability for your meals. Many clients initially shy away from making appointments with dietitians, thinking that they will force you to eat more (or less) than you are comfortable with. However, most dietitians who specialize in working with people who have eating disorders are very patient and able to move at a pace you are comfortable with. Regular physician visits are also extremely important for anyone suffering from an eating disorder. Your physician will be able to assess if you are experiencing any medical complications from your disorder and can determine if you need a higher level of care with additional medical monitoring. Many sufferers also see a physician who specializes in mental illnesses, a psychiatrist. This is the person who will prescribe and monitor any medications to help with the symptoms you are experiencing. Who Should Get Outpatient Therapy? If you dont have any medical complications that need to be monitored, and if youre able to make progress on an outpatient basis, then outpatient therapy for eating disorders may work well for you. For instance, if youre able to follow a meal plan with only the support of your family, then outpatient treatment may be a good fit. However, if you arent able to follow a meal plan at all, or you have medical complications or also suffer from depression, a higher level of care may be more appropriate. Most people suffering from an eating disorder use outpatient treatment options at some point in their recovery. They may only be treated on an outpatient basis or they may attempt recovery on an outpatient basis only to decide later that a higher level of care is necessary. Even people who are treated at a higher level of care typically continue treatment on an outpatient basis after they complete work with the additional support of intensive outpatient, inpatient or residential treatment, so you (or your family member) likely will be treated on an outpatient basis at some point during recovery. Maslowââ¬â¢s motivation theory shows that individuals are being motivated by unfulfilled needs (Clifford Thorpe, 2007). According to Maslow, there are five types of needs. These needs are illustrated in the figure below. Maslow Theory of Motivation ââ¬â Hierarchy of Needs (tutor2u) Maslow identified these five needs which are considered the ââ¬Å"deficiency needs. â⬠When one is motivated to satisfy these needs, one will work hard to achieve it. Therefore, these needs must be satisfied in order for work productivity to be improved. When these needs are satisfied, naturally, employees would want to remain in the company. Thus, employers can motivate individuals by satisfying these five types of needs mentioned by Maslow differently. For physiological motivation, employers can provide sufficient meal breaks for employees. This is done in the hope that employees can not only have sufficient time to eat a proper meal, but also time for them to rest and to be re-energized. This time can also be used for the employees to run their errands, since most shops are often closed for the day when these employees knock off. To meet the employeesââ¬â¢ safety needs, employers can provide a safe working environment that is generally free of threats to the employees. In this way, employees would feel at ease during work as their safety is being well taken care off. If the employees have job security, it also enhances their satisfaction. For social needs, employers have to ensure that employees do feel a sense of belonging with the company. In order to do so, employers can maintain traditions and working characteristics to create a sense of belonging and of shared values and objectives. By using all means of communications, employers can create a relaxed and collaborative mood between all workers, and also to encourage a team game and a sense of responsibility in the employees. However, this would also mean transferring some of the control from the employers to the employees. For esteem motivators, employers can recognize employeesââ¬â¢ achievements by appreciating their work in forms of incentive pay systems. Other than monetary forms, employers can also recognize their efforts in the companyââ¬â¢s annual dinners by showing their appreciation with medals or trophies that enhance the rewarded employeeââ¬â¢s status, allowing them to feel valued and appreciated for their hard work. Lastly, for self-actualization, employers can offer more challenging and meaningful work assignments which would require employees to be creative and innovate, so that they can progress with accordance to their long-term goals. However, not every individual is motivated by the exact same needs. Employers have to recognize such needs that each employee is pursuing and try to fulfill these needs with reasonable solutions that would benefit both parties. Thus, with employers trying their best to satisfy these five needs as mentioned by Maslow constantly, employees would be more motivated to continue working with the company as they feel valued and appreciated by employers. Thus, this shows that not only monetary incentives are able to motivate employees, the basic needs of an individual are equally essential for employees to fulfill as well. According to Frederick Herzbergââ¬â¢s two factory motivation theory, individuals are affected by two factors; motivation factors which include personal satisfaction and psychological growth and hygiene factors like dissatisfaction. Some typical hygiene factors that Herzberg discovered during his investigation in the U. S. A. would be working conditions, feelings of security, job, interpersonal relation and status (Management. net, 2009). Therefore, if these hygiene factors are not sufficient to the employees, they can be turned into the sources of dissatisfaction with work. For employeesââ¬â¢ personal growth, motivation factors like recognition for achievement, responsibility of tasks and advancement to higher level tasks are needed as they are able to create job satisfaction. These factors can help to motivate an individual to perform and achieve above-average performance and effort. However, all hygiene factors must be met first in order for motivation factors to be fulfilling. The different combination of hygiene factors and motivation factors results in four scenarios. A high hygiene but low motivation factor results in an environment where employees have few complaints but are not highly motivated because they merely work for a living. If employees are motivated and yet still have complaints, this shows that the job is full of excitement and challenging, but the wages and working conditions does not meet the minimal individual requirements. The worst situation is the combination of low hygiene and low motivation where unmotivated employees have lots of complaints. Of course, the perfect situation is the combination of high hygiene and high motivation where employees are highly motivated and have fewer complaints (Tutor2u. net, 2010). Herzberg suggested that employers should focus on the people-oriented approach which emphasizes on job satisfaction through job rotation, an arrangement where employees are able to engage in a variety of jobs in the same organization to increase interest and motivation, and also to discover which is the most suitable job for them. In addition, job enrichment is yet another method where employers can enhance individual jobs, allocating more responsibilities to individuals with the aim of rewarding and inspiring the staff. A combination of the previous two methods is likely to result in the best result (Fisher, Schoenfeldt, Shaw, 2003). In todayââ¬â¢s work and other contexts, motivation does not necessary mean using the traditional approach of monetary incentives. Other incentives, for instance, free travel packages, movie tickets or other impromptu recognition rewards are also able to motivate employees to high performance work levels, often without the usage of large monetary incentives (Tan CH, D Torrington, 2004). In ââ¬Å"The Human Side of Enterpriseâ⬠, Douglas McGregor examined theories of individual behaviors at work, thus formulating two theories which can be categorized as Theory X and Theory Y (Accel-Team, 2010). According to McGregor, Theory X assumes workers work mainly for money and security, and dislike work and would avoid it if possible. Thus, in order for Theory X workers to achieve organizational objectives, various kinds of rewards offered by employees would be the most popular and appropriate motivator to such employees. However, on the other hand, Theory Y assumes there are workers who enjoy challenges in their work and would want more from their jobs than just mere monetary incentives. Therefore, for Theory Y workers, employers have to create a creative working environment or culture in order for employees to show and develop their creativity (Tutor2u. net, 2010). Under Theory X, employers must be careful to maintain a delicate balance between the two extremes ââ¬â hard and soft approach towards employees. The hard approach refers to management using coercion, micromanagement, and reining a tight control over the employees, while a soft approach relies on taking a permissive and harmonious approach towards them, in the hope that they will cooperate when required. However, McGregor asserts that using such approaches are not the most efficient because it might result in hostility from employees if the former approach is used, or cause repeated requests for various benefits when a soft approach is used to entice the employees. On the other hand, under the Theory Y, employeesââ¬â¢ personal goals can be aligned with organizational goals by using the employeeââ¬â¢s own quest for fulfillment as the motivator (NetMBA. com, 2007). McGregor stressed that the soft management approach does not apply to Theory Y management and it is also recognized that some individuals may not be able to reached the level of maturity assumed by Theory Y and therefore, tighter controls can be gradually reduced as employees develops their independence. The discussion thus far leads to the conclusion that people with more money tend to be happier than those with less ââ¬â but only up to a certain extent. Moneyââ¬â¢s impact on happiness suffers from diminishing returns: once an individual have sufficient food and shelter, more cash does not bring much extra joy (Buchanan, 2009). Researchers believe that extra capital leave people less worried about their financial health, and are thus less stressed. However, ââ¬Ëexperimental experiencesââ¬â¢ such as trips to the theatre or travel trips sponsored by employers brings more happiness and joy to employees rather than just monetary incentives. These types of clauses can modify the whole sentence, as well as verbs, adverbs, and adjectives, and may show aspects such as time, reason, concession, or condition. These clauses may often start with words such as while, if, because, when, although, unless, since, so that, whereas, even if, in case, as long as, and other words. In contrast, an adjectival clause will modify a noun and start with a relative pronoun (that, who, whose, whom, or which) or a subordinate conjunction (whenà andà where). Facts on Mass Shootings in the US On Oct. 1, 2017, the Las Vegas Strip became the site of the deadliest mass shooting in American history. A shooter murdered 59 people and injured 515, bringing the victim total to 574.à If it seems as if the problem of mass shootings in the U.S. is getting worse, thats because it is. Heres a look at the history of mass shootings to explain the historical and contemporary trends. Definition of Mass Shootingà First, its important to define this type of crime. A mass shooting is defined by the FBI as a public attack, distinct from gun crimes that happen within private homes, even when those crimes involve multiple victims, and from drug- or gang-related shootings. Historically, through 2012, a mass shooting also has been considered a shooting in which four or more people were shot. In 2013, a new federal law reduced the figure to three or more. The Frequency of Mass Shootings Increasing Every time a mass shooting occurs, a debate is spurred in the media about whether such shootings are happening more often than they used to. The debate is fueled by a misunderstanding of what mass shootings are. Some criminologists argue that they are not on the rise because they count them among all gun crime, a relatively stable figure year-over-year. However, considering mass shootings as defined by the FBI, the disturbing truth is that they are rising and have increased sharply since 2011. Analyzing data compiled by the Stanford Geospatial Center, sociologists Tristan Bridges and Tara Leigh Tober found thatà mass shootings have progressively become more common since the 1960s. Through the late 1980s, there were no more than five mass shooting per year. Through the 1990s and 2000s, the rate fluctuated and occasionally climbed as high as 10 per year. Since 2011, the rate has skyrocketed, climbing first into the teens then peaking at 473 in 2016, with the year 2018 ending at a total of 323 mass shootings in the U.S. Number of Victims Rising Data from the Stanford Geospatial Center, analyzed by Bridges and Tober, shows that the number of victims is rising along with the frequency of mass shootings. The figures for deaths and injuries climbed from below 20 in the early 1980s to spike sporadically through the 1990s to 40 and 50-plus and reach regular shootings of more than 40 victims through the late 2000s and 2010s. Since the late 2000s, there have been 80-plus to 100 deaths and injuries in some mass shootings. Most Weapons Legally Obtained; Many Were Assault Weapons Mother Jonesà reportsà that of the mass shootings committed since 1982, 75 percent of the weapons used were obtained legally. Among those used,à assault weapons and semi-automatic handguns with high-capacity magazinesà were common. Half of the weapons used in these crimes were semi-automatic handguns, while the rest were rifles, revolvers, and shotguns. Data on weapons used, compiled by the FBI, shows that if the failed Assault Weapons Ban of 2013 had been passed, the sale of 48 of these guns for civilian purposes would have been illegal. Uniquely American Problem Another debate that crops up in the media following a mass shooting is whether the U.S. is exceptional for the frequency at which mass shootings occur within its borders. Those who claim that it does not often point to Organization for Economic Co-operation and Development (OECD) data which measures mass shootings per capita based on a countrys total population. Looked at this way, the data indicates that the U.S. ranks behind nations including Finland, Norway, and Switzerland. However, this data is based on populations so small and events so infrequent that its statistically invalid. Mathematician Charles Petzold explains on his blog why this is so, from a statistical standpoint, and further explains how the data can be useful. Instead of comparing the U.S. to other OECD nations, which have much smaller populations than the U.S. and most of which have had just one to three mass shootings in recent history, compare the U.S. to all other OECD nations combined. Doing so equalizes the scale of population and allows for a statistically valid comparison. This indicates that the U.S. has a mass shooting rate of 0.121 per million people, while all other OECD countries combined have a rate of just 0.025 per million people (with a combined population three times that of the U.S.). This means that the rate of mass shootings per capita in the U.S. is nearly five times that in all other OECD nations. This disparity is not surprising given thatà Americans own nearly half of all civilian guns in the world. Mass Shooters Nearly Always Men Bridges and Tober found that of the mass shootings that have occurred since 1966, nearly all were committed by men. Just five of those incidents- 2.3 percent- involved a lone woman shooter. That means men were the perpetrators in nearly 98 percent of mass shootings. Connection Between Mass Shootings and Domestic Violence Between 2009 and 2015, 57 percent of mass shootings overlapped with domestic violence, in that the victims included a spouse, former spouse, or another family member of the perpetrator, according to an analysis of FBI data conducted by Everytown for Gun Safety. Additionally, nearly 20 percent of attackers had been charged with domestic violence.à Assault Weapons Ban Would Reduce Problem The Federal Assault Weapons Ban was in effect between 1994 and 2004. It outlawed the manufacture for civilian use of some semi-automatic firearms and large capacity magazines. It was prompted into action after 34 children and a teacher were shot in a schoolyard in Stockton, California, with a semi-automatic AK-47 rifle in 1989 and by the shooting of 14 people in 1993 in a San Francisco office building, in which the shooter used semi-automatic handguns equipped with a hellfire trigger, which makes a semi-automatic firearm fire at a rate approaching that of a fully automatic firearm. A study by The Brady Center to Prevent Gun Violence published in 2004 found that in the five years prior to the bans implementation, assault weapons it outlawed accounted for nearly 5 percent of gun crime. During its period of enactment, that figure fell to 1.6 percent.à Data compiled by the Harvard School of Public Health and presented as a timeline of mass shootings shows that mass shootings have occurred with much greater frequency since the ban was lifted in 2004, and the victim count has risen steeply. Semi-automatic and high-capacity firearms are the weapons of choice for those who perpetrate mass shootings. Asà Mother Jonesà reports, more than half of all mass shooters possessed high-capacity magazines, assault weapons, or both. According to this data, a third of the weapons used in mass shootings since 1982 would have been outlawed by the failed Assault Weapons Ban of 2013.
